Potential camp counselors, please complete the following:

2. Have your Pastor complete a Reference on you by clicking on the Reference Form button and Send them the link.
3. Have two additional references complete the Reference Form.

4. Download and fill out the Background Investigation Consent form.
5. Download and fill out the Health Form.
6. Send both the Background Investigation Consent form and the Health form to:
